In this test, an allergen is placed on the skin and brought into contact with the epidermis using a small needle tip. The development of redness or swelling on the skin within approximately 15–20 minutes indicates the presence of an allergy to the relevant substance.

The prick test is usually performed on the arm or back and can be safely applied to all age groups, including children. Antihistamine medications must be discontinued before the test; otherwise, the results may be affected. The procedure is painless and does not leave permanent marks.

The test panel includes common allergens such as pollens, house dust mites, molds, animal dander, and foods. The results help identify the cause of the allergy and support the creation of an appropriate treatment plan. When necessary, immunotherapy or environmental precautions may be recommended.

Mild itching or redness after the prick test is normal and usually resolves on its own in a short time. It is important not to scratch the area after the procedure and to follow the doctor’s recommendations. The test results make it possible to develop personalized approaches in allergy management.

What You Need to Know

Information

Name of the Procedure

Prick Test

Application Area

Skin – usually the forearm or back area

Purpose of Use

Identification of substances (allergens) that cause allergies

Application Method

A very small amount of allergen is dropped onto the skin, then the skin surface is lightly pricked with a fine needle

Procedure Duration

Results can be observed within 15–20 minutes

Preparation Before the Test

Antihistamine medications should be stopped at least 7 days before the test

Pain / Discomfort

A mild pricking sensation may occur; generally painless

Frequency of Application

Performed once when allergy diagnosis is needed; may be repeated if necessary

Evaluation of Results

Redness and swelling at the allergen application site indicate a positive result

Risks / Side Effects

Mild itching and redness may occur; rarely, a severe allergic reaction may develop

Who Is It Suitable For

Children and adults showing allergy symptoms

Alternative Methods

Blood test (specific IgE), patch test, intradermal test

Treatment According to Results

Avoidance of identified allergens, antihistamine treatment, immunotherapy (allergy vaccine) may be recommended

Specialist Dr. Seval Karasatı

Dr. Seval Karasatı is a dermatology (skin diseases) specialist in Ankara specializing in skin health, hair and nail diseases, aesthetic dermatology, and laser applications. class="ez-toc-section" id="What_Is_a_Prick_Test">What Is a Prick Test?

A prick test is a fast and reliable skin test used in the diagnosis of allergic diseases. Suspected allergen substances are applied to the skin as small droplets, and a superficial prick is made over each one. It is usually performed on the inner forearm. The appearance of redness or swelling within 15–20 minutes indicates an allergy to the relevant substance. It is widely used to detect allergens such as foods, pollen, and house dust.

If you are allergic to a substance, special immune cells just under your skin called “mast cells” wait there equipped with IgE antibodies specific to that allergen, functioning like tiny “antennas.” During the test, when that allergen enters your skin, it binds to these antennas like a key fitting into a lock.

This binding triggers an alarm mechanism in the mast cells, causing them to rapidly release the chemicals inside them, especially histamine.

The histamine released enlarges the blood vessels in that area, causing fluid to leak out and stimulating nerve endings, which leads to itching.

What we clinically observe as a result of this is two main findings that appear within 15 to 20 minutes after the test is applied: first, a pale raised bump in the center (swelling), and then a redness (flare) surrounding this bump. When evaluating the allergy test result, what we measure and assess is the diameter of this central swelling.

What Types of Allergies Can This Prick Test Detect?

This test is used to detect immediate reactions in which IgE antibodies play a role. In most cases, they are the result of a complex interaction between this genetic background and environmental factors. For dermatologists, the prick test is an indispensable diagnostic tool in identifying these “environmental,” meaning external, triggers. In cases where we cannot determine the cause of a rash or itching just by looking at the skin itself, this test shows us whether there is an underlying allergic mechanism.

How Does This Allergy Test Guide Us in Atopic Dermatitis (Eczema)?

Atopic dermatitis (AD), commonly known as eczema, is a very common chronic and itchy skin disorder. A significant proportion of these patients also have an allergic predisposition. When prick testing is applied to patients with atopic dermatitis, the positivity rate has been observed to be quite high (up to around 77%).

However, there is one critical finding here: the overwhelming majority of these positive results in AD patients (more than 66%) are against environmental aeroallergens.

This finding gives us a very important clue for the management of eczema patients. Instead of immediately starting very restrictive and difficult diets whenever the patient has symptoms, focusing the test panel particularly on common environmental triggers such as house dust mites, pollens, and animal dander usually gives more accurate results. If significant sensitivity to these environmental allergens is detected in testing, taking measures to reduce them at home (mite-proof mattress covers, HEPA-filter vacuum cleaners, etc.) can contribute far more to disease control than dietary restrictions.

What Does a Prick Test Investigate in Patients With Chronic Urticaria (Hives)?

Chronic urticaria (hives) is another common skin disease characterized by itchy wheals lasting longer than six weeks. In many cases, no clear underlying cause is found, which is why it is called “idiopathic.” However, prick testing can also be used in these patients to understand whether there is an allergic trigger.

Similar to eczema, when prick test positivity is detected in chronic urticaria patients, most of these positive results (more than 41%) are again due only to aeroallergens. The rate of positivity related only to foods is quite low in this group (around 7%). This suggests that chronic hives, too, are often more related to environmental factors or the body’s internal mechanisms than to food allergy.

Are Standard Allergy Tests (Prick Test Solutions) Always Sufficient?

The allergen solutions (extracts) we use in tests are standardized commercial products in order to provide reliable and repeatable results. These solutions contain the proteins of the allergen source (for example pollen or peanut) and are stored under special conditions (usually between +2°C and +8°C).

However, this standardization process has an important limitation: not all allergenic proteins are stable. Some allergenic proteins, especially those found in fresh fruits and vegetables, are “labile,” meaning they are sensitive to heat, processing, or chemical procedures. The commercial extract preparation process can destroy the structure of these fragile proteins and cause them to lose their allergenic properties.

This may lead a patient who is allergic to a fresh apple to have a false-negative result in a prick test performed with a standard “apple extract.” In other words, the patient is allergic, but the test does not show it.

What Is “Prick-to-Prick” (Allergy Testing With Fresh Food), and Why Is It Done?

This is exactly why, in situations where standard extracts are insufficient, we resort to a method called “Prick-to-Prick” (PTP), in which the food itself is used. This test is especially critical for diagnosis in fresh fruit, vegetable, and some raw seafood allergies.

As its name suggests, the PTP technique is a direct “from food to skin” application. The procedure has two stages: first, a sterile lancet is inserted into the fleshy or juicy part of the suspected fresh food. Then, that same lancet, which has come into contact with the food, is used to prick the patient’s skin, just like a standard prick test, so that the allergen superficially enters the skin.

If a dry food such as hazelnut, peanut, or walnut is suspected, it is first crushed into powder and then mixed with a few drops of saline to make a paste. This paste is placed on the patient’s skin, and the lancet is applied through the paste into the skin. The result is read in 15–20 minutes, just like the standard test.

Contact Us for Anything You Wonder About Skin Health!

 Contact via WhatsApp

In Which Special Situations Is the Prick-to-Prick Test Used?

This method is particularly valuable in the following situations:

Suspicion of Oral Allergy Syndrome (Pollen-Food Syndrome)

Suspicion of fresh fruit allergy (apple, peach, cherry, kiwi, etc.)

Suspicion of fresh vegetable allergy (carrot, celery, tomato, etc.)

Raw nut allergies

Situations where the standard commercial test is negative, but the patient history (reaction after eating the food) is positive

Distinguishing tolerance between raw, cooked, or baked forms in milk and egg allergies

How Is the Prick-to-Prick Test Interpreted Clinically?

The information this test provides is extremely valuable. If a patient’s test with a standard allergen extract (SPT) is negative, but the prick-to-prick test using the fresh form of the same food is positive, this clearly tells us the following: the patient’s allergy is directed against a labile protein that has broken down in the commercial extract.

This is usually an indication of what is known as “Pollen-Food Syndrome” or “Oral Allergy Syndrome.” In this syndrome, the patient is often sensitive to pollen and also reacts to a labile protein in fresh fruit that resembles the protein in the pollen.

The good news for these patients is this: these labile proteins usually cause local symptoms such as itching in the mouth or lip swelling rather than severe systemic reactions like anaphylaxis. More importantly, since these proteins break down with heat, patients can often tolerate these foods without problems when they are cooked.

Which Medications Must Be Stopped Before Coming for an Allergy Test (Prick Test)?

This is the most critical rule for the accuracy of the test. Many medications used in allergy treatment block the effect of histamine, which is the mechanism on which the test works. If the test is performed while taking these medications, the test site may not swell even if you are allergic, resulting in a false-negative result.

For the test to be accurate, some medications that block histamine must be stopped for a certain period before testing. The discontinuation period varies depending on the strength of the medication. The main ones are:

Short-acting H1 antihistamines: at least 3 days (72 hours)

Long-acting H1 antihistamines: 5 to 7 days

Very strong/long-acting H1 antihistamines: 2 weeks (14 days)

Tricyclic antidepressants with strong antihistaminic effects: 7 to 14 days

H2 blockers (some stomach medications): 24 to 48 hours

Systemic (oral) corticosteroids (even short-term use may require discontinuation; the doctor should be consulted)

If I Cannot Stop My Chronic Medications (Such as Antidepressants), Can I Still Have a Prick Test?

This is an important issue our patients frequently encounter. In some cases, it may not be medically possible or safe for the patient to stop a strongly antihistaminic medication for 1–2 weeks. If these essential medications cannot be discontinued, the prick test cannot be performed reliably.

However, this does not mean that allergy cannot be diagnosed. In such patients, we use alternative in vitro diagnostic methods that are not affected by medication use, namely specific IgE blood tests.

Which Medications Can I Continue to Use, Since They Do Not Affect the Prick Test?

Fortunately, many common medications do not affect the test result and do not need to be stopped. Some widely used medications that do not affect the test and do not need to be discontinued are:

Corticosteroid nasal sprays (generally)

Asthma inhalers

Montelukast-containing medications

Most antibiotics

Simple pain relievers such as paracetamol

Blood pressure, diabetes, and thyroid medications

Who Cannot Undergo a Prick Test?

Some situations make the test too risky or impossible to perform. The prick test procedure is extremely quick and causes minimal discomfort. It is usually applied to the inner forearm or, in small children or where there is no room on the arm, to the back.

After the test area is cleaned, the allergen drops to be tested are placed on the skin with numbering or markings. At least 2 cm of space should be left between each drop. This rule is critical to prevent reactions from mixing together if two neighboring drops are positive.

After the allergen is applied, a sterile, single-use lancet is passed through the drop to lightly puncture the most superficial layer of the skin. This should not cause bleeding; the goal is only to allow the allergen to come into contact with skin cells. The procedure is not painful, at most causing a slight pricking sensation or mild itching a few minutes after the test begins.

What Are the “Control” Drops in an Allergy Test Used For?

In every prick test application, two control drops must be used to ensure the validity of the test. Without these controls, the results cannot be interpreted:

Positive Control (Histamine): This drop contains pure histamine, the main chemical of the allergic reaction. Its purpose is to confirm that the patient’s skin is capable of reacting and is not under the influence of antihistamines. The histamine drop must produce a wheal. If it does not, the entire test is considered invalid and must be repeated.

Negative Control (Saline/Solvent): This drop contains saline or the preservative solution in which allergens are diluted, without containing any allergen. Its purpose is to determine whether the reaction is due to the allergen itself or simply to the mechanical trauma of pricking. The negative control should not swell.

When and How Are Prick Test Results Read?

The results should be read 15 to 20 minutes after the prick procedure. This period is required for histamine release and the resulting swelling to reach its maximum level.

When evaluating the result, the focus is not the surrounding redness but the central wheal itself. In clinical practice, the most commonly accepted and reliable method is to measure the longest diameter of the wheal with a ruler, then measure the diameter perpendicular to it, and record the average of these two values in millimeters.

What Result Is Considered “Positive” in an Allergy Test?

There are internationally accepted criteria for a result to be classified as “positive.” For a reaction to be considered positive:

The average diameter of the wheal must be at least 3 millimeters (mm) or larger.

And this wheal must be at least 3 mm larger than the wheal caused by the negative control, if any.

And of course, for all these results to be valid, the positive control (histamine) must also have produced a valid reaction.

Does a Positive Prick Test Mean “I Have an Allergy”?

This is the most commonly misunderstood and most important point for both patients and sometimes clinicians: no, it does not necessarily mean that by itself.

A positive test means there is “sensitization” to that substance, meaning your immune system has recognized it and produced IgE antibodies against it.

This is not the same thing as a “clinical allergy.” A clinical allergy means that when you encounter that substance, you experience symptoms such as coughing, runny nose, itching, shortness of breath, or hives.

Many people may be “sensitized” to certain substances despite having no symptoms. Diagnosis of allergy is made when the test result and the patient’s history match perfectly.

Is the Size of the Wheal Important in an Allergy Test?

Yes, it is very important. One of the most important factors strengthening the relationship between sensitization and clinical allergy is the size of the wheal.

The larger the diameter of the wheal, the higher the likelihood that the patient will experience symptoms when exposed to that allergen.

While the standard 3 mm threshold determines technical positivity, it may not always be sufficient for clinical correlation. For example, a 3 mm positivity for a food may have low clinical significance, whereas an 8 mm positivity is highly likely to indicate clinical allergy. For aeroallergens such as house dust mites or grass pollen, a wheal reaching 10 mm is strong evidence that the probability of clinical reactivity is very high.

Is the Prick Test Safe, and Are There Any Risks?

When performed properly, the prick test is generally considered a very safe procedure. The most common side effect is itching and swelling at the test sites for a few hours, which is already an expected part of the test.

However, since it introduces the allergen directly into the body, even in a very small amount, there is always a theoretical risk of triggering a serious systemic allergic reaction. This risk is especially a little higher in those with a history of severe food or insect venom allergy.

That is exactly why the prick test must be performed in a clinical setting where emergency equipment and medications are immediately available, by trained healthcare personnel.

What Should Be Considered After the Allergy Test?

After the test results are read, there are a few simple points to keep in mind:

Remain under observation in the clinic for 20–30 minutes, especially if you had positive results or if you have asthma.

Expect the itching and swelling at the test sites to decrease on their own within 1–2 hours.

After the results are read and your doctor approves, you may apply a cold compress to the test sites if the itching bothers you a lot.

Again, with your doctor’s approval, you may restart the antihistamine medication that you stopped so it would not affect the test.

What Should Be Done If a Serious Allergic Reaction (Anaphylaxis) Develops During the Test?

This is the rarest but most serious scenario and requires the healthcare team to be prepared. Anaphylaxis is a rapidly progressing, life-threatening medical emergency. There is a clear order of priority in treatment:

The life-saving medication is adrenaline (epinephrine). If the patient develops breathing difficulty, low blood pressure, or widespread rash, adrenaline is immediately administered intramuscularly.

Antihistamines and corticosteroids are supportive treatments. These medications do not correct life-threatening respiratory distress or circulatory collapse. Therefore, delaying treatment to give antihistamines first is a dangerous mistake; adrenaline must always be the priority.

In Summary, Why Is the Prick Test Such a Valuable Diagnostic Method?

The Skin Prick Test is a rapid, reliable, cost-effective, and fundamental diagnostic tool in the evaluation of dermatologic conditions such as Atopic Dermatitis (eczema) and Chronic Urticaria (hives). Its reliability depends on the patient correctly discontinuing antihistamine medications before the test, sometimes for as long as two weeks. Prioritizing environmental allergens in eczema patients and using the Prick-to-Prick method in suspected fresh food allergy increases diagnostic accuracy. Although a wheal of 3 mm or larger is considered “positive,” the larger the wheal, the greater the chance that it reflects clinically meaningful symptoms. Since it carries a rare but real risk of anaphylaxis, it should be performed in a clinical environment prepared for emergency epinephrine intervention, and the results must always be interpreted together with the patient’s clinical history.

Frequently Asked Questions

How reliable is the prick test in determining the type of allergy?

The prick test has a high accuracy rate because it directly measures the allergic response in the skin.
