This method reduces the size of ink particles by applying targeted energy to tattoo pigments and removes them from the skin surface through the natural healing process.

Q-Switched or Pico laser technologies effectively penetrate tattoo pigments in different colors. Thanks to short and intense energy pulses, the pigments are broken into microscopic pieces without being exposed to heat. In this way, the risk of burns, scarring, or pigmentation on the skin is kept to a minimum.

Laser tattoo removal is applied in sessions that vary according to the tattoo’s color, depth, and the type of ink used. This process, usually completed in 4 to 10 sessions, is safely performed with professional devices and under the supervision of a dermatologist.

Mild redness or crusting may be seen on the skin after the procedure; this is temporary and heals quickly with proper care. Protecting the area from the sun, using moisturizer, and not picking the scabs support skin renewal and reduce the risk of scarring.

Things You Should Know

Information

Procedure Name

Laser Tattoo Removal

Application Area

All areas of the body with unwanted tattoos

Purpose

Gradual removal or reduction of the visibility of permanent tattoos from the skin

Technology Used

Q-Switched Nd:YAG laser

Mechanism of Action

The laser light targets the tattoo pigments and breaks them into small pieces; The laser light is adjusted to focus not on the skin itself but only on the dye pigment (that is, the color) inside the tattoo. The tattoo pigment absorbs this intense light energy in a very small fraction of a second and suddenly reaches a very high temperature. This sudden energy load causes the pigment to virtually “break apart and explode.”

The most critical point here is that this heat must be trapped only inside the tattoo pigment and prevented from spreading to the surrounding healthy skin tissue. To achieve this, the pulse duration of the laser must be shorter than the time required for the pigment to cool down without spreading its heat outward. If the laser pulse lasts too “long” (and here we are talking about one billionth of a second), the heat spilling over from the pigment can burn the skin. This may lead to scarring or permanent pigmentation changes. This is exactly why the main goal of modern tattoo removal technologies is based on making pulse durations as short as possible.

What is the difference between picosecond and nanosecond (Q-Switched) lasers in tattoo removal?

There are two main laser technologies used in tattoo removal, and the difference between them is based on how short their pulse durations are and therefore on their working principles.

Nanosecond (NS) Lasers (Q-Switched):

These are the “traditional” Q-Switched systems that have been used for many years and can deliver pulses as short as one billionth of a second (nanosecond). These lasers primarily work through a “photothermal” effect, that is, by heat. The laser pulse heats the pigment very quickly, and this thermal shock breaks the dye particle into smaller pieces.

Nanosecond lasers are especially effective in the first sessions of treatment and on large pigment particles. However, as the treatment progresses, the tattoo pigments become increasingly smaller. The “cooling time” of these microscopic particles also becomes dramatically shorter. The fixed pulse duration of the nanosecond laser (for example 30–50 nanoseconds) begins to remain “longer” than the cooling time of these tiny particles. As a result, the heat can no longer be trapped inside the particle, spreads to the surrounding area, and the risk of burning healthy skin increases. Therefore, the final stages of treatment with nanosecond lasers become both less effective and higher in complication risk.

Picosecond (PS) Lasers:

These are the most modern technologies that can deliver pulses as unimaginably short as one trillionth of a second (picosecond). Picosecond lasers work more with a “photomechanical,” that is, shockwave effect, rather than a heat effect.

The pulse duration is so short that there is almost no opportunity for the tattoo pigment to heat up. Instead, the laser energy creates enormous mechanical stress inside the pigment, that is, a very powerful “shock wave” (sound wave). This shock wave breaks the pigment apart not by heating it, but by striking and vibrating it. It virtually turns the tattoo ink into “dust.” This process is sometimes also called “cold fragmentation,” because there is no accumulation of heat spreading to the surrounding tissue.

In summary; nanosecond lasers “heat and explode” the dye, while picosecond lasers “turn the dye into dust with a shock wave.” This mechanical effect makes picosecond systems both much safer (lower risk of burns and pigmentation) and more effective (especially in small and stubborn pigments).

Are picosecond lasers really better in tattoo removal?

Yes, current clinical evidence and scientific studies clearly support that picosecond technology is the current “gold standard.” It has significant advantages over nanosecond systems:

Picosecond lasers can break tattoo pigment into much smaller pieces (almost like “dust”) than nanosecond lasers can. It is much easier for the body’s immune system (macrophages) to clear these very small dust particles than larger particles. This generally enables faster and more complete pigment clearance in tattoos treated with picosecond lasers.

This more efficient fragmentation also generally helps reduce the total number of treatment sessions.

Its clearest advantage is its safety profile. Since picosecond lasers work more through mechanical effect than heat damage, the heat spreading to the surrounding tissue is minimal. This significantly reduces the risk of post-procedure pigmentation (PIH – Post-inflammatory Hyperpigmentation) compared to nanosecond lasers. The reduction in heat damage means a serious decrease in the risk of brown pigmentation especially seen in wheatish or dark-skinned individuals.

In addition, patients generally describe picosecond laser treatments as less painful than nanosecond lasers because the heat effect is minimal. This is also an important factor that increases patient compliance and comfort during treatment.

Which laser head is used for different tattoo colors?

The idea that “one laser removes all colors” is not correct. Effective removal of multicolored tattoos requires the use of different laser wavelengths (different heads) appropriate to the absorption profile of each dye pigment.

1064 nm (Nd:YAG Laser):

This is the most frequently used wavelength because of the prevalence of black ink. It is extremely effective for black and dark blue pigments. It has been proven that the picosecond 1064 nm laser is significantly superior to all other lasers (including nanosecond 1064 nm) in clearing black ink. Its most important feature is that it shows the least absorption by melanin in the upper layer of the skin (the substance that gives skin its color) and can penetrate the deepest. This feature makes it the safest and only preferred choice for dark-skinned patients (Fitzpatrick IV–VI), reducing the risk of thermal damage.

532 nm (Nd:YAG, Frequency-Doubled):

This wavelength is ideal for red ink and related warm colors (orange, yellow, purple). The picosecond 532 nm laser is much more effective in red tattoos than nanosecond alternatives.

Green and Blue Pigments (Challenging Colors):

In the past, green and blue pigments were considered “stubborn” colors and required special lasers such as Q-Switched Ruby (694 nm) or Alexandrite (755 nm). However, these older systems carried a higher risk of pigmentation. Current clinical findings show that the picosecond 532 nm laser, thanks to its high shockwave capacity, is also extremely successful in clearing green pigments that were previously very difficult to treat. This means that for green color, the old and risky systems are no longer necessary.

Contact Us for Anything You Wonder About Skin Health!

 Contact via WhatsApp

What is evaluated during the consultation for tattoo removal treatment?

The tattoo removal process is not a standard package treatment; it is a medical procedure that must be planned entirely on an individual basis. During the initial consultation, we carefully evaluate many factors in order to determine the success of the treatment, the number of sessions, and the possible risks.

The main factors taken into account in this evaluation are as follows:

Your skin type (Fitzpatrick score)

The location of the tattoo (which part of the body it is on)

The age of the tattoo

The type of tattoo (amateur or professional)

Whether it is a cover-up tattoo

The colors of the ink used

The density and depth of the ink

Your general health condition

Your wound healing history (history of keloid or hypertrophic scar)

Why is Skin Type So Important?

The most critical factor in the evaluation is your skin tone. The darker your skin is (wheatish, dark), the more melanin (natural pigment) is present in the upper layer of the skin. The laser light may not be able to distinguish between the tattoo ink and this melanin. This causes the laser to target the skin’s own pigment instead of the tattoo, increasing the risk of post-procedure pigmentation (brown spots – PIH) or color lightening (white spots – hypopigmentation). To prevent this risk in dark-skinned patients, the 1064 nm wavelength must be preferred and treatment should proceed with lower, more cautious energy levels. This may increase the total number of sessions somewhat.

How Does the Tattoo Location Affect the Number of Sessions?

The job of the laser is to “break up” the tattoo; the actual “cleanup” is done by the body’s immune system. Areas with strong blood circulation and lymphatic drainage (the immune system’s cleaning network) respond more quickly. Tattoos on areas close to the heart and with good circulation, such as the head, neck, trunk, and upper arms, fade more quickly. Tattoos in distal areas where circulation is weaker (ankles, fingers) are removed much more slowly and always require more sessions.

Other Factors:

Old tattoos (10+ years) are generally easier to remove because the body’s immune system has already partially broken down the pigments over the years. Amateur tattoos are generally more superficial and contain less ink, so they are easier to remove. Professional tattoos, on the other hand, are both deeper and much denser, making them more difficult. The most difficult cases are “cover-up” tattoos; because they contain a double load of ink and require many more sessions.

Why are tattoo removal sessions performed at such long intervals?

This is one of the issues patients are most curious about. It is generally recommended to wait 6 to 8 weeks, and sometimes even 10–12 weeks, between traditional tattoo removal sessions. The reason for this period is not only to allow the skin to heal.

The laser’s job is to break up the ink during the session. After the laser leaves the room, the body’s immune system takes over the actual work. The cleaning cells called “macrophages” perceive those microscopic dye residues fragmented by the laser as “waste,” engulf them (phagocytosis), and remove them from the area through the lymphatic system (the body’s waste drainage system).

This biological “debris removal” process takes time. A minimum period of 6–8 weeks is needed for the skin to recover and for the macrophages to complete their task by clearing the fragmented pigment from the area. Doing a session earlier (for example every 3–4 weeks) does nothing but unnecessarily traumatize the skin and increase the risk of complications. Because in this case, the same area is targeted again while the body has not yet cleaned up the debris from the previous session.

Is it possible to accelerate the tattoo removal process? (PFD Method)

The biggest limitation of traditional treatment is the momentary “whitening” or “frosting” that occurs on the skin when the laser pulse is delivered during the session. This whitening is actually caused by the microscopic gas bubbles (cavitation) created on the skin surface by the laser. These bubbles act like a mirror, reflecting subsequent laser beams back and creating an “optical shield.” Until this shield dissipates (which takes 20 minutes in the traditional R20 method), another effective pulse cannot be delivered to the same area.

This is where the “R0” (zero waiting) or “PFD” (Perfluorodecalin) method comes into play. Perfluorodecalin is a special medical-grade liquid used to break this optical shield and is generally applied in a transparent patch.

Immediately after the first laser pulse, this PFD patch is applied to the area. PFD rapidly dissolves those reflective gas bubbles in approximately 5 seconds and removes the optical barrier. This allows the doctor to perform 3–4 consecutive laser passes (multi-pass) on the same area in the same appointment. Much more ink is fragmented in a single session.

Clinical studies have shown that this accelerated approach, especially when combined with picosecond lasers, significantly reduces the total treatment time and the number of sessions compared to traditional methods. However, this multi-pass protocol means that more intense energy is delivered to the skin, so it must be performed by physicians experienced with powerful modern systems.

Contact Us for Anything You Wonder About Skin Health!

 Contact via WhatsApp

Are there risks or side effects to tattoo removal?

Although modern picosecond laser systems have made tattoo removal much safer, this is still a medical procedure and carries potential risks. Some side effects may be seen after the procedure:

The most commonly encountered conditions are as follows:

Post-procedure brown pigmentation (PIH)

Lightening of skin color (Hypopigmentation)

Blistering (bulla or blister)

Crusting

Temporary redness and edema

Bruising (especially in areas close to vessels)

Itching

Most of these are a normal and temporary part of the healing process. However, there are also more serious and potentially permanent risks:

Pigment (Color) Changes:

These are the most common delayed complications. Hyperpigmentation (PIH) is the skin darkening in response to the procedure and is especially seen in dark-skinned individuals or those who do not protect themselves from the sun after the procedure. It is generally temporary but may last for months. Hypopigmentation, on the other hand, consists of white spots resulting from the skin losing its natural pigment. This condition may be more difficult to correct and is generally associated with the wrong choice of laser wavelength (for example 532 nm on dark skin). Since picosecond lasers reduce heat damage, they have significantly reduced these pigmentation risks (especially PIH) compared to nanosecond systems.

Risk of Scarring:

This is the most undesirable but irreversible risk. Scarring is generally caused by excessive thermal damage, that is, using higher energy (fluence) than the skin can tolerate, or by the patient forcibly removing the scabs formed after the procedure. The low heat load of picosecond technology has minimized this risk, but not eliminated it.

What is the risk of ‘paradoxical darkening’ in cosmetic tattoos (permanent makeup)?

This is a very important and specific risk that requires special attention. It is especially seen in light-colored cosmetic tattoos (pink, flesh-toned, or white), such as permanent lip makeup, eyebrow contouring, or scar camouflage tattoos.

The inks of these types of cosmetic tattoos generally contain metallic compounds such as titanium dioxide or iron oxide. When laser energy (both nanosecond and picosecond) encounters these metallic pigments, it may cause a chemical reaction (reduction). This reaction can suddenly turn a pink, white, or flesh-colored pigment into jet black or dark gray. This is called “paradoxical darkening” (PDI).

This risk is related not to the brand of the laser but entirely to the chemistry of the ink. There is only one way to manage this risk: before starting full treatment, a mandatory test pulse must be performed on a small, inconspicuous area of the tattoo. The response after the test pulse is observed. If darkening occurs, the treatment plan is completely changed. (This newly formed black pigment may be tried again with a 1064 nm laser, but this significantly prolongs and complicates the process.)

What should I pay attention to after the tattoo removal procedure?

The success of the treatment depends as much on post-procedure care as on the laser procedure itself. Following the rules during the healing process is the key to reducing the risk of side effects (pigmentation or scarring) to zero.

The basic rules that should be followed after the procedure are as follows:

Keep the area clean and dry.

Use the reparative creams or antibiotic ointments recommended by your physician regularly for the specified period.

If you are advised to cover the area with a sterile dressing, change the dressing at the specified frequency.

The most important rule: Absolutely do not pick, peel, or scratch the scabs that form.

(Forcibly removing the scabs is the number one reason for permanent scarring or pigment changes.)

If blisters (bullae) form, do not burst them and do not peel the skin covering them.

Protect the area strictly from sunlight and tanning beds for at least 4–6 weeks.

When going outside, be sure to apply SPF 50+ (high protection factor) mineral-filter sunscreen to the area.

Protect the area from trauma (friction, impact, tight clothing).

Avoid hot water, Turkish baths, saunas, and chlorinated pools for the first few days.

What should I expect from tattoo removal treatment?

Current scientific evidence has established picosecond laser systems as the “first-choice” and “gold standard” treatment in tattoo removal. The reason for this recommendation is that picosecond lasers, thanks to their superior shockwave mechanism, provide statistically faster and more complete clearance, while also being a much safer option with less risk of pigmentation and less pain.

To maximize clinical efficiency and shorten treatment time, combining multi-wavelength picosecond technology with the R0/Perfluorodecalin (PFD) protocol (patch method) represents the current best practice by providing maximum fragmentation in a single session.

Clinical success depends on a comprehensive pre-assessment and a meticulous protocol. The most important point that should not be forgotten is that tattoo removal is not a magic wand but a biological process based on the body’s immune system. For this reason, the tattoo’s location (blood circulation), age, and the type of ink used continue to remain important factors determining the number of sessions, just as much as the technology used.

Frequently Asked Questions

How does laser tattoo removal work?

Laser tattoo removal breaks down tattoo pigments with high-intensity light pulses and enables them to be eliminated by the skin over time.
