Sun rays, hormonal changes, aging, and post-inflammatory reactions are among the most common causes. Treatment is planned according to the type, depth, and skin type of the spot.

Topical treatments are the first step in combating skin pigmentation spots. Products containing hydroquinone, azelaic acid, kojic acid, retinoids, and vitamin C suppress melanin production and even out the skin tone. Regular use provides noticeable lightening of color in the long term.

Professional dermatological applications are effective for resistant and deep-seated spots. Chemical peeling, microdermabrasion, laser treatment, and IPL applications stimulate skin renewal and reduce pigment density. These procedures should be performed under the supervision of a specialist physician.

Post-treatment skin care and protection are critically important to prevent the formation of new spots. The use of high-protection-factor sunscreen, avoiding direct sun exposure, and care with antioxidant-containing dermocosmetic products increase treatment success.

Things You Should Know

Information

Treatment Area

Dermatology – Skin pigmentation treatment

Targeted Problem

Sun spots, melasma, age spots, post-acne hyperpigmentation, birthmarks

Causes of Pigmentation Spots

Sun exposure, hormonal changes, genetic factors, skin trauma

Methods Used

Laser treatments, chemical peeling, mesotherapy, PRP, medical creams

Laser Treatment

The color of spots is lightened and skin tone is evened out with Q-Switched Nd:YAG laser, IPL, and fractional laser systems

Chemical Peeling

The appearance of spots is reduced by controlled peeling of the upper skin layer with acids such as TCA and glycolic acid

Mesotherapy

Pigment production is balanced by injecting vitamins, antioxidants, and spot-lightening substances under the skin

PRP Application

Skin renewal is supported through the injection of plasma obtained from the person’s own blood

Topical Creams

Products containing ingredients that lighten skin tone and reduce pigment production (hydroquinone, kojic acid, vitamin C)

Sun Protection

The use of high-factor sunscreen is mandatory throughout the entire treatment period

Treatment Duration

Depending on the applied method and the depth of the spot, it may take from several sessions to months

Suitable Patient Profile

Appropriate treatment options are available for all individuals, women and men, with different skin types

Side Effects

Temporary redness, flaking, and mild crusting may occur; class="ez-toc-section" id="What_are_skin_pigmentation_spots_and_what_are_the_most_common_types">What are skin pigmentation spots and what are the most common types?

The color of our skin is determined by melanin pigment produced by special cells called “melanocytes.” The more this production increases, the darker our skin tone becomes. When this delicate balance is disrupted by various factors such as sun rays (UV), hormonal fluctuations, or the skin’s response to an injury, melanocytes begin to produce excessive amounts of pigment in certain areas. This condition is called “hyperpigmentation,” in other words, pigmentation.

Although there are a wide variety of pigmentation types clinically, each requiring a different approach, in practice there are three main types of pigmentation that we encounter most frequently and that determine our treatment protocols:

Melasma (Chloasma): Generally known as the “pregnancy mask,” melasma consists of broad, symmetrical, irregularly bordered brown patches that appear on the face, especially on the forehead, cheeks, and upper lip area. It is overwhelmingly seen in women and is directly associated with hormonal fluctuations (pregnancy, use of birth control pills). Sun exposure is the number one factor that triggers melasma and rapidly worsens the existing condition. The response to treatment varies depending on which layer of the skin the pigment is located in; superficial (epidermal) pigmentation responds better to treatment, while deep (dermal) pigmentation is much more stubborn.

Solar Lentigo (Sun Spots): These spots, also popularly known as “age spots,” are actually not related to age but directly related to cumulative sun damage accumulated over the years. They are generally sharply bordered, small, brown, freckle-like spots that appear in individuals over the age of 50 and on the areas most exposed to the sun throughout life (the backs of the hands, face, shoulders, and arms). These are benign formations but are an indicator of chronic sun damage.

Post-Inflammatory Hyperpigmentation (PIH): This type of pigmentation is a direct result of trauma or inflammation experienced by the skin. When the skin is damaged (such as acne, burn, cut, eczema, rash, or an aggressive cosmetic procedure), it may produce excessive pigment in that area as part of the healing process. Especially squeezing pimples, scratching wounds, or picking scabs almost guarantees the formation of this type of pigmentation. Although PIH affects every skin type, it tends to be much more common, darker in color, and longer-lasting in dark-skinned individuals.

In pigmentation management, there are also other conditions that need to be considered in addition to these three main categories.

The main pigmentation problems considered in pigmentation treatment are:

Melasma (Hormonal pigmentation)

Solar Lentigo (Sun and age spots)

Post-Inflammatory Hyperpigmentation (Acne and scar pigmentation)

Freckles (Triggered by genetic predisposition and sun)

Birthmarks

Pigmentations caused by certain medications

How does my skin type affect my pigmentation treatment risk?

One of the most critical factors determining success and safety in pigmentation treatment is the person’s skin type. In clinical practice, we use a classification system that measures the skin’s response to the sun (the tendency to burn or tan). This scale ranges from very fair-skinned individuals who always burn (Type I) to very dark-skinned individuals who never burn (Type VI).

This classification is of vital importance for risk analysis, especially before starting procedural treatments such as laser, light systems, or chemical peeling. The reason is very clear: the pigment cells (melanocytes) of dark-skinned individuals (Type IV, V, and VI – from wheat-colored skin to black skin) are genetically much more “reactive,” that is, sensitive.

This means: any controlled injury, heat, or energy applied to the skin (for example, a laser pulse) can easily stimulate these sensitive cells. The skin may perceive this intervention as an “attack” and produce new and darker pigmentation (Post-Inflammatory Hyperpigmentation – PIH) in that area as a defense mechanism. In other words, while trying to treat the pigmentation, there is a risk of making it even worse with an incorrect intervention.

Because of this high risk, the pigmentation treatment protocol in dark-skinned patients should always be much more “conservative,” that is, gentle and careful.

The main risks and requirements that are taken into consideration when planning treatment for dark skin types (Type IV, V, and VI) are:

The high risk of developing new pigmentation (PIH) after the procedure

The extreme sensitivity of pigment cells (melanocytes)

Low tolerance to procedures such as lasers and peels

The possibility of treatment burns turning into permanent pigmentation

The necessity of lower energy levels and a gentle treatment protocol

The mandatory need for pre-treatment skin preparation with suppressive treatment (slowing pigment production)

Why is sun protection the most important step in pigmentation treatment?

The absolute cornerstone of all pigmentation treatment protocols, the most critical component that must not be compromised, is sun protection. Even the most expensive, most aggressive laser or cream treatments performed without sun protection are doomed to fail or for the pigmentation to return rapidly. The number one factor triggering pigmentation formation is UV rays; they constantly send a “produce more” signal to pigment cells.

However, in pigmentation treatment we need to go beyond standard sunscreens. Clinical evidence confirms that not only UVA and UVB rays but also High Energy Visible Light (HEVL), that is, blue light (emitted from the sun, phones, tablets, and computer screens), triggers pigment production (melanogenesis) and plays an important role especially in the recurrence of melasma.

The clinical meaning of this is: classic, colorless sunscreens that provide only UV protection are generally insufficient for managing pigment disorders and especially melasma.

Our protocol requires specially formulated sunscreens containing iron oxide. These are the products popularly known as “tinted sunscreens.” The iron oxide pigments in these products act almost like a physical shield against visible light and blue light, preventing them from reaching the skin. Studies have shown that tinted protectors containing iron oxide provide much superior results in preventing melasma flare-ups compared to colorless UV filters.

For this reason, recommending a tinted, iron oxide-containing product ceases to be a general skincare recommendation and becomes a critical “treatment intervention” that addresses the entire light spectrum triggering pigmentation formation.

The features that should be present in an ideal sunscreen for pigmentation patients:

Broad spectrum (UVA and UVB protection)

At least SPF 30 (ideally SPF 50+)

Iron Oxide

Pigmented Titanium Dioxide (Physical/Mineral filter)

Zinc Oxide (Physical/Mineral filter)

Antioxidants (providing additional protection such as vitamins C and E)

Contact Us for Anything You Wonder About Skin Health!

 Contact via WhatsApp

Which creams are used as the first step in pigmentation treatment?

Topical treatment, that is, cream protocols, is always the first step in pigmentation management and offers high effectiveness by directly intervening in the pigment production process.

In this field, Hydroquinone (HQ), generally in concentrations of 2% to 4%, is considered the worldwide “gold standard” among spot-lightening agents. Its primary mechanism is the inhibition of “tyrosinase,” the key enzyme in melanin synthesis. Simply put, it turns off the main switch of the pigment production line and has a suppressive effect on pigment cells.

For the optimal management of melasma, instead of hydroquinone alone, Triple Combination Creams (TCC), which show superior efficacy through synergistic effect, are often preferred. These formulations work from three different directions to maximize treatment success.

The contents of triple combination creams (TCC) generally consist of these three main components:

Hydroquinone (The main spot-lightening substance)

Tretinoin (A Retinoid – Skin renewal agent)

A mild Corticosteroid (Irritation preventive)

In this combination, Tretinoin (Retinoid) increases the turnover rate of skin cells. This helps the existing pigmented upper skin shed more quickly and also allows hydroquinone to penetrate the skin better. The corticosteroid, on the other hand, minimizes the redness, burning, and irritation that the other two strong components may cause, thereby increasing the tolerability, that is, the sustainability, of the treatment.

Very Important Safety Warning: Risk of Ochronosis

The continuous use of products containing Hydroquinone (HQ) (alone or within TCC), even under dermatologist supervision, must definitely be limited to a maximum of six months. The reason for this strict limitation is the risk of a serious side effect called “exogenous ochronosis.” Ochronosis is a permanent, nearly impossible-to-treat bluish-black or gray-blue speckled discoloration that develops in the treated areas of the skin.

Because of this risk, after the 6-month intensive treatment phase, it is mandatory to immediately switch to hydroquinone-free, safe “maintenance” treatments to prevent recurrence of the pigmentation.

What spot-lightening cream options are available other than Hydroquinone (HQ)?

Due to the risks of long-term use of Hydroquinone (HQ), we need safe and effective alternatives to manage pigmentation treatment throughout life. These agents are used both during the mandatory breaks when hydroquinone must be stopped and for maintaining the success achieved after the initial treatment (maintenance treatment). In chronic conditions such as melasma, we protect the gains achieved with HQ by switching to these agents.

These agents suppress pigment production more gently through different mechanisms:

Azelaic Acid (15% – 20%): It is a very effective and safe alternative with the ability to inhibit the tyrosinase enzyme (the key to the pigment factory). It also has anti-inflammatory properties, which makes it an excellent option especially for post-acne pigmentation (PIH) and skin prone to rosacea.

Niacinamide (Vitamin B3): It targets pigmentation in a very intelligent way. Instead of stopping melanin production, it physically blocks the “transfer” of melanin packages (melanosomes) produced in the pigment cell to the surrounding skin cells. In other words, even if pigment is produced, it makes it harder for it to become visible on the skin surface.

Topical Tranexamic Acid (TXA): Its topical forms prevent the stimulation of pigment cells, especially by interrupting the inflammatory signaling chain (plasminogen activation) triggered by UV rays.

Retinoids (Especially for PIH): Especially in the management of acne-related PIH (post-inflammatory pigmentation), synthetic retinoids such as Adapalene or Tazarotene provide excellent results by gently renewing the skin and dispersing the pigment.

The main safe ingredients used in long-term maintenance treatment or as an alternative to HQ are:

Azelaic Acid (15-20%)

Niacinamide (Vitamin B3)

Topical Tranexamic Acid

Vitamin C (L-Ascorbic Acid)

Retinoids (Adapalene, Tretinoin, Tazarotene)

Arbutin (A herbal hydroquinone derivative)

Kojic Acid

Resorcinol

If cream treatments are insufficient, are oral medications used?

Yes, especially for stubborn (refractory) melasma cases that do not respond to cream treatments, peels, and other methods, systemic, that is, oral treatments constitute the next step. Our most effective and safe option in this field is Oral Tranexamic Acid (TXA) treatment.

Oral tranexamic acid is a medication normally used to reduce or stop bleeding (anti-fibrinolytic). However, its effect on pigmentation works through a completely different mechanism: it blocks the signaling pathway called “plasminogen activator,” which starts in skin cells after UV light, hormones, or injury and stimulates pigment cells. In other words, it cuts off at the source the signal telling the pigment cell to “produce.”

Moreover, tranexamic acid has another very important additional benefit: it is also thought to reduce the abnormal vascular development (angiogenesis) and the associated redness frequently seen beneath melasma lesions. This enables it to target both the pigmentary (brown) and vascular (red) components of melasma at the same time.

The dermatological dose used for melasma (generally 250 mg two or three times a day for 12 weeks) is much lower than the doses used for bleeding disorders. Therefore, the risk of serious side effects such as thromboembolic events (embolism, deep vein thrombosis) is extremely rare. However, before starting this treatment, the patient must be comprehensively questioned and screened for clotting risk factors.

The conditions that must definitely be questioned before starting oral tranexamic acid treatment and that prevent the use of this medication are:

A history of past clotting events (venous or arterial thromboembolism)

Active vascular occlusion

Known inherited clotting disorders (Thrombophilia)

A history of ischemic heart disease or stroke

Severe kidney failure

Active or past certain types of cancer

What role does chemical peeling play in pigmentation treatment?

Chemical peeling is an important method used “in addition to” cream treatments in order to accelerate the process of clearing superficial epidermal pigment (pigmentation in the upper layer of the skin). It is very important to understand that peeling is an “accelerator”; it is not a permanent treatment on its own, but when applied correctly, it greatly increases the effectiveness of cream treatments.

Peeling removes the affected layers of the skin by creating a controlled chemical injury and allows fresher, less pigmented, and improved-texture new skin to emerge from below.

However, in order for chemical peeling to be used safely in pigmentation treatment, the “pre-procedure skin preparation” protocol is mandatory. Especially in dark-skinned individuals, performing peeling without preparing the skin first (that is, without suppressing pigment cells with agents such as hydroquinone or retinoids) may mean inviting new pigmentation (PIH) instead of treating the existing pigmentation. The skin must be “calmed” with these preparatory creams for at least 2-4 weeks, and only then should the peeling procedure be applied.

The peeling agents most frequently used in pigmentation treatment are:

Glycolic Acid (AHA – Superficial)

Salicylic Acid (BHA – Superficial, for oily skin and PIH)

Jessner’s Solution (Superficial)

Trichloroacetic Acid (TCA – Medium depth)

Resorcinol

Lactic Acid (AHA – Gentler superficial)

While superficial peels (AHA, BHA) are safe for epidermal melasma and PIH, medium-depth TCA peels may be used for more stubborn pigmentation, but their risks increase in dark skin. Deep chemical peels (such as Phenol), however, are absolutely not used for pigmentation treatment in dark skin due to the risk of serious scarring and permanent color change.

Contact Us for Anything You Wonder About Skin Health!

 Contact via WhatsApp

Are laser treatments effective and safe for pigmentation spots?

Lasers are the most powerful but also the riskiest tools in pigmentation treatment. The correct device, correct dose, and correct patient selection are of vital importance. Wrong laser selection or incorrect parameter use may permanently worsen the pigmentation.

Sun and Age Spots (Solar Lentigo): These spots respond extraordinarily well to treatments that target only epidermal (superficial) pigment. Lasers such as IPL (Intense Pulsed Light) and Q-Switched 532 nm “see” and eliminate this superficial pigment very effectively.

Melasma and Post-Acne Pigmentation (PIH): This is the most difficult and riskiest area for laser treatment. In these two conditions, the basic rule is to minimize thermal damage (heat). Because heat is the main factor triggering these spots; heat = inflammation = new pigmentation (PIH).

For this reason, in the treatment of melasma and PIH, we must turn to gentler technologies that minimize heat:

Long Wavelengths (Nd:YAG 1064 nm): Q-Switched 1064 nm Nd:YAG lasers, thanks to their longer wavelength, pass through the pigment in the upper layer of the skin without becoming “caught” and focus on the deeper dermal pigment. Because they do not unnecessarily heat the skin surface, they are considered one of the safest laser options especially for dark skin types (Type IV-VI).

Ultra-Short Pulse Durations (Picosecond Lasers): The revolution in pigmentation treatment came with picosecond technology. Traditional lasers (Nanosecond) broke down pigment by “heating” it (photothermal effect). This heat always carried a PIH risk in sensitive skin.

Picosecond lasers, on the other hand, use incredibly short pulse durations (one trillionth of a second) and send a mechanical “shock wave” (photoacoustic effect) to the pigment instead of heating it. They mechanically break the pigment down into dust-like particles without causing thermal damage. This is a much safer and more effective method, especially in melasma and dark-skinned patients, as it allows the pigment to be eliminated without triggering an inflammatory response and the subsequent pigmentation (PIH).

The main laser and light systems selected according to the type of pigmentation and skin tone:

IPL (Only for superficial sun spots, risky in melasma)

Q-Switched 532 nm Laser (Only for superficial sun spots)

Q-Switched 1064 nm Nd:YAG (For dermal melasma and dark skin)

Picosecond Lasers (The safest technology for melasma, PIH, and dark skin)

Fractional Lasers (Generally used in combination to open microchannels in the skin and increase the absorption of topical medications [such as TXA])

What should be considered to prevent the spots from returning?

This is perhaps the most important question in pigmentation treatment. Melasma is a chronic (lifelong) skin condition with no definitive cure and a very high tendency to relapse (recur). Our success in treatment is measured not by eliminating the spots in one session, but by how closely the patient adheres to the lifelong “maintenance” strategy.

Effective pigmentation management always follows certain stages:

Preparation Stage: If a procedure is planned, preparing the skin with suppressive creams for 2-4 weeks.

Intensive Clearing Stage: Maximally reducing the appearance of the spots with hydroquinone creams (maximum 6 months), and if necessary, additional laser, peeling, or oral TXA treatments.

Maintenance Stage: After stopping HQ as required, switching to safe hydroquinone-free agents to prevent relapse and making sun protection a lifestyle.

This final stage, the “Maintenance Stage,” is permanent. There are basic rules that must be followed for life in order to prevent the spots from returning:

The cornerstones of the lifelong maintenance protocol are:

Daily, uncompromising use of tinted sunscreens containing iron oxide

Avoiding direct sun exposure during the hours when the sun is strongest (10:00-16:00)

Wearing wide-brimmed hats and sunglasses

Continuous use of non-HQ brightening agents (Azelaic acid, Niacinamide, Vitamin C, etc.)

Gentle cleansers and moisturizers that keep the skin barrier strong

Avoiding harsh scrubs, loofahs, or aggressive cosmetic products that may irritate the skin

Being careful about hormonal triggers (for example birth control pills)

Frequently Asked Questions

Which methods provide the most effective results in skin pigmentation treatment?

Among the most effective methods in skin pigmentation treatment are laser applications, spot mesotherapy, chemical peeling, PRP treatment, and medical creams.
