Psoriasis, which develops on a genetic basis triggered by environmental factors, is a multisystemic and chronic inflammation process characterized by a dramatic shortening of the skin cell renewal cycle as a result of erroneous signals from the immune system. After the clinical diagnosis made by a specialist physician, the process is a long-term treatment partnership that includes not only the elimination of skin lesions but also a holistic approach to possible accompanying psoriatic arthritis, metabolic syndrome, and cardiovascular risks.

Specialist Dr. Seval Karasatı

Dr. Seval Karasatı is a dermatology (skin diseases) specialist in Ankara specializing in skin health, hair and nail diseases, aesthetic dermatology, and laser applications. class="ez-toc-section" id="What_Does_Psoriasis_Change_in_Our_Body">What Does Psoriasis Change in Our Body?

To understand psoriasis, we must first imagine how healthy skin works. Under normal conditions, our skin is like a perfectly functioning factory. New skin cells (keratinocytes) produced in the deepest layer slowly mature and move toward the surface. In a healthy person, this journey takes approximately 28 to 30 days. At the end of this period, the cells complete their lifespan and shed from the skin in a way that is not visible to the eye. We do not even feel this cycle.

However, in psoriasis patients, this factory begins to work at an out-of-control speed. As a result of a misunderstanding in the immune system, our defense cells constantly send a “produce faster” signal to the skin. That calm cycle, which should normally take a month, is compressed into as little as 3 to 5 days.

As a result of this tremendous speed, the cells do not have the opportunity to mature. These living cells, which cannot be shed from the surface, pile up on top of one another. Those silver-colored, thick scales you see on your skin are actually a pile of unshed and accumulated skin cells. The redness underneath is the result of enlarged blood vessels trying to carry blood to this rapid production.

Some triggers that play a role in the onset of this process are:

Intense stress

Physical trauma

Infections

Smoking

Alcohol consumption

Some medications

How Common Is Psoriasis in Turkey and Who Does It Affect?

When we look at the data from our country, we see that psoriasis affects a significant portion of our society. The prevalence in Turkey ranges between 1 and 3 percent. Although this rate may seem small, when compared to our population, we estimate that approximately one to one and a half million people in our country live with this disease.

We also know that the disease has a geographical preference; its prevalence increases as you move away from the equator and closer to the poles. However, our country’s genetic pool and climate also have their own unique characteristics. The disease generally peaks in two age periods; the first is young adulthood between the ages of 15 and 30, and the second is after the late 50s. However, this does not mean that this disease cannot be seen in babies or at very advanced ages.

It is a fair disease in terms of gender; it affects women and men at almost equal rates. However, in our clinical practice, we observe that the age of onset is slightly earlier in female patients and that the psychological burden is felt more heavily by women due to aesthetic concerns. In addition, the low humidity and cold weather brought by the winter months in Turkey significantly increase our patients’ complaints.

Which Type of Psoriasis Might You Have?

Not every psoriasis patient presents with the same picture. The disease has different “faces” depending on the site of involvement, the shape of the rash, and its extent. Knowing which type you have is critical for determining our treatment roadmap. These types vary from the most common to the rarest.

The type we encounter most frequently, which constitutes the vast majority of cases, is “Plaque Psoriasis.” Here, we see sharply demarcated red areas with silvery scales on them, on the knees, elbows, scalp, and sacral region. However, sometimes the rashes can also appear in very different and unexpected parts of the body.

The different clinical forms in which the disease may be seen are:

Plaque psoriasis

Guttate psoriasis

Inverse psoriasis

Palmoplantar psoriasis

Erythrodermic psoriasis

Pustular psoriasis

Nail psoriasis

The “Guttate” form, which we especially see in children and young people, usually breaks out suddenly after a throat infection and covers the body with small raindrop-like eruptions. The “Inverse” type favors fold areas such as the armpits and groin and, due to moisture, does not cause scaling, but is only bright red. The form involving the palms and soles can seriously reduce the patient’s quality of life by making walking and using the hands difficult.

Contact Us for Anything You Wonder About Skin Health!

 Contact via WhatsApp

How Does Your Doctor Diagnose Psoriasis and What Signs Do They Look For?

Many of my patients worry, thinking that complex tests or biopsies will be required for diagnosis. However, the diagnosis of psoriasis is largely “clinical”; in other words, the observation of an experienced dermatologist is often sufficient for diagnosis. While examining you, we do not just look at the rashes, we examine the character of those rashes.

There are some special examination findings we rely on during diagnosis. For example, when we gently scrape the scales on the lesion, their lifting like a “wax stain” or, when scraped a little more, the appearance of pinpoint bleeding spots (Auspitz sign) is very typical for us.

In addition, we definitely examine your nails. Because nail involvement is seen in half of psoriasis patients. Small pits the size of a pinhead on the nails, separation of the nail from the nail bed, or yellowish discoloration under the nail may be signs of psoriasis.

The changes we may observe in the nails are:

Pitting

Onycholysis

Oil spot appearance

Subungual hyperkeratosis

Nail destruction

Does Psoriasis (Comorbidities) Affect Only the Skin?

This is one of the issues that surprises my patients the most. Seeing psoriasis as merely a “cosmetic skin problem” unfortunately means underestimating the seriousness of the disease. In modern medicine, we accept psoriasis as a systemic inflammation state that concerns the whole body. The immune cells that cause that redness on the skin can actually circulate through the blood and affect your other organs as well.

About one-third of our patients may develop what we call “Psoriatic Arthritis.” It manifests itself with pain in the joints, swelling, a feeling of stiffness in the mornings, and sausage-like swelling in the fingers. If you have psoriasis and your lower back, heels, or hand joints hurt, you should definitely share this with us.

In addition, there is a concept we call the “Psoriatic March.” Severe psoriasis may prepare the ground for arteriosclerosis in the long term. This increases the risk of heart diseases, diabetes mellitus, high blood pressure, and obesity.

Other health problems that may accompany psoriasis are:

Psoriatic arthritis

Obesity

Type 2 diabetes

Hypertension

Dyslipidemia

Inflammatory bowel diseases

Depression

Anxiety

How Is the Severity and Extent of the Disease (PASI Score) Determined?

Before starting treatment, we need to draw the “map” of your disease. To say “mild,” “moderate,” or “severe,” we do not rely on our personal opinions but on mathematical scoring systems accepted all over the world. These scores are also determining criteria for the Social Security Institution (SSI) in Turkey to cover treatments.

The method we use most frequently is the PASI score (Psoriasis Area Severity Index). In this calculation, the darkness of the redness on your body, the thickness of the skin, the intensity of scaling, and the affected area are scored.

In addition, as a practical method, we use the “Body Surface Area” (BSA) calculation. We accept the patient’s (that is, your) palm, including your fingers, as 1% of your body surface area. If your rashes cover a total area larger than 10 of your palms, this indicates that the disease is severe and that systemic treatment may be required.

However, physical measurements do not mean everything. Questionnaires measuring the impact of the disease on your mood and social life (DLQI – Dermatology Life Quality Index) are also very effective in our treatment decision.

The scales we use to determine disease severity are:

PASI Score

Body Surface Area

Dermatology Life Quality Index

Physician Global Assessment

Contact Us for Anything You Wonder About Skin Health!

 Contact via WhatsApp

Which Creams (Topical Treatment) Are Used in the First Step of Treatment?

In treatment protocols in Turkey, we apply the principle of “stepwise treatment.” If your disease is mild or moderate, our first weapon is always topical treatments, namely creams, ointments, and lotions. It is very important for you to be patient at this stage, because using creams regularly requires discipline.

Cortisone-containing (steroid) creams are the medications we use most often to quickly extinguish inflammation. However, you do not need to label them as “dangerous” and be afraid; when used under a doctor’s supervision, for the correct duration and dose, they are extremely safe. To reduce the risk of side effects in long-term use, we generally switch to intermittent methods such as “weekend therapy.”

In addition to steroids, creams containing vitamin D derivatives are also very effective because they slow down cell proliferation. Sometimes we also prepare mixtures containing salicylic acid to dissolve the scales. In areas where the skin is thin, such as the face or groin, we prefer newer-generation medications that do not contain cortisone.

The topical agent groups we use in the first step are:

Topical corticosteroids

Vitamin D analogues

Keratolytics

Third-generation retinoids

Calcineurin inhibitors

Moisturizers

Tar preparations

When Does Light Therapy (Phototherapy) Come Into Play When Creams Are Insufficient?

If we cannot achieve sufficient success with creams or if your rashes have spread over a very large area of your body, we move on to the second step: Phototherapy. Please do not confuse this with a solarium. While solariums contain uncontrolled rays that age the skin, in phototherapy we apply in hospitals, only rays with very specific wavelengths that have healing properties are used.

In Turkey, we most frequently apply “Narrowband UVB” treatment. In this method, you come to the hospital 2 or 3 days a week and receive light treatment in special cabins for periods measured in seconds or minutes. This light suppresses the excessive immune response in the skin.

In more resistant cases, we may use PUVA treatment. In this method, you take a medication beforehand or enter water containing the medication to increase the effect of the light, and then the light is applied. Phototherapy is an extremely effective option with a low side effect profile, especially in patients with widespread plaques.

Phototherapy methods are:

Narrowband UVB

Broadband UVB

PUVA

Bath PUVA

Local Phototherapy

Who Are Oral Treatments (Systemic Agents) Suitable For?

If the disease follows a resistant course, does not respond to phototherapy, or if joint involvement accompanies it, we must now treat it not from the outside but from the inside. At this point, the drugs in pill or injection form that we call “conventional systemic agents” come into play. These drugs are our reliable friends that we have used for years and whose effects and side effects we know very well.

One of the drugs we prescribe most often is Methotrexate. It is used only once a week and slows down excessive cell proliferation. To prevent side effects, we definitely give folic acid vitamin along with it.

If a rapid improvement is required, we may use a drug called Cyclosporine. It is very powerful, but due to its effects on blood pressure and the kidneys, we generally prefer it for short-term use (during crisis periods).

Another option is Acitretin, a vitamin A derivative. It is especially very effective in hand-foot psoriasis, but its use is very limited in women of childbearing age because it can cause serious problems in the baby in case of pregnancy. All of these drugs require regular blood test monitoring.

The main drugs used in systemic treatment are:

Methotrexate

Cyclosporine

Acitretin

What Are Biological Drugs and Injection Therapy, and Who Are They Given To?

Thanks to developments in the world of science, we now have biological treatments that we can call “smart missiles.” If none of the classical treatments (cream, light, pill) have worked or cannot be used because of side effects, we switch to these high-tech drugs.

Biological drugs target and block the specific molecules that cause the disease (such as TNF, Interleukin-17, Interleukin-23). They are generally in the form of injections administered under the skin. Their effectiveness is quite high; in most of our patients, we can see that the skin becomes completely or almost completely clear.

However, in Turkey, there are strict rules determined by the Ministry of Health and the SSI for starting these drugs (SUT Criteria). First of all, you must have tried the other treatments and failed. In addition, since these drugs suppress immunity at a specific point, we screen you in detail for infections such as tuberculosis and hepatitis before treatment.

The mandatory screenings performed before biological treatment are:

Chest X-ray

PPD test or Quantiferon test

Hepatitis B and C tests

HIV test

Complete blood count

Liver function tests

How Is Psoriasis Managed During Pregnancy and in Children?

One of the biggest concerns of women with psoriasis is, “What will happen if I get pregnant?” or “Will my medications harm my baby?” The good news is that psoriasis generally tends to calm down during pregnancy along with hormonal changes. However, flare-ups may be seen after childbirth together with stress and sleeplessness.

During pregnancy, the safety of the baby is more important than anything. For this reason, we stop many oral medications. Our safest harbor is moisturizers and low-dose creams. If necessary, we can safely use Narrowband UVB light therapy, which does not reach the baby because it does not mix into the bloodstream.

In children, however, the situation is a little different. In little ones, we usually see sudden eruptions after throat infections. In treatment, our priority is always the mildest option; namely creams. In persistent cases, light therapy or systemic medications in doses suitable for children are planned in cooperation with pediatric specialists.

The safe approaches preferred in special situations are:

Intensive moisturizer support

Low/Medium potency topical steroids

Narrowband UVB phototherapy

Cyclosporine when necessary

Biological agents with appropriate pregnancy category

What Should You Pay Attention to While Living with Psoriasis?

Treatment is not only about the prescription your doctor writes. The changes you make in your lifestyle are at least as effective as medications. In the outpatient clinic, I always tell my patients this: “I will do my best, but you must also take good care of your body.”

Obesity in particular is one of psoriasis’s greatest enemies. Fat tissue works like an organ that produces inflammation in the body and fuels psoriasis. Losing weight both increases the effectiveness of medications and reduces the severity of the disease.

Smoking is definitely a red line for psoriasis patients. It not only triggers the disease, but is also the biggest cause of psoriasis involving the palms and soles (pustular psoriasis). It also reduces the success of the medications we use in treatment.

Stress management, healthy nutrition, and regular exercise will be your greatest helpers on this long journey.
