In this condition, also known among the public as ala disease, the affected areas become completely colorless and take on a chalk-white appearance because pigment production stops. Vitiligo, which is not contagious but develops through the complex interaction of genetic predisposition, oxidative stress, and autoimmune mechanisms, is not just a cosmetic color change but a biological and immunological process that must be managed with the correct medical approaches.

Specialist Dr. Seval Karasatı

Dr. Seval Karasatı is a dermatology (skin diseases) specialist in Ankara specializing in skin health, hair and nail diseases, aesthetic dermatology, and laser applications. class="ez-toc-section" id="What_Is_Vitiligo_and_Why_Does_It_Occur_in_the_Body">What Is Vitiligo and Why Does It Occur in the Body?

To understand the logic of vitiligo, we can liken our skin to a huge paint factory. In this factory, there are special worker cells called “melanocytes,” and their job is to produce the paint called melanin. In the vitiligo process, the body’s immune system (that is, the defense soldiers), as a result of a misunderstanding, perceives these paint workers as enemies and begins to attack them. As a result of this attack, either the workers die or they become unable to produce by becoming frightened and hiding. As a result, in those areas where pigment is not produced, our skin loses its natural color and milk-white patches appear.

Regarding why this process begins, in the medical world we focus on several main theories. The strongest theory is the autoimmunity I mentioned above; that is, the confusion of the immune system. In addition to this, a condition we call “oxidative stress” is also very effective. You can think of it like iron rusting; metabolic waste accumulating in your cells disrupts the structure of melanocytes, making them more fragile and leading to their death. Of course, there is a genetic predisposition, but this does not mean “my mother has it, so I will definitely have it too” or “it will definitely be passed on to my child.” Family history is present in only one-fifth of patients. Stress, severe grief, sunburns, or physical traumas may be the last straw pulling the trigger in genetically predisposed individuals.

Into Which Types Is Vitiligo Divided and Why Is This Distinction Important?

In order to manage the disease correctly, we must first determine which type of vitiligo we are dealing with. Because each type has a different character, and the treatment strategy we will apply changes completely accordingly. Vitiligo is basically divided into two main groups according to its pattern of spread.

Non-Segmental Vitiligo is the form we encounter most frequently. In this type, white spots generally show a symmetrical distribution on both sides of the body. In other words, if a spot appears on your right hand, there is a high probability that a spot will also appear in a similar location on your left hand. It generally follows a slow but progressive course. It pauses from time to time, and from time to time it may flare up and spread with stress or illness.

Segmental Vitiligo, on the other hand, follows a different and interesting course. It generally begins suddenly in childhood or adolescence. It progresses in the form of a strip or band along a nerve line on only one half of the body. It does not pass to the other side. It starts very rapidly, spreads quickly over 6 months to 2 years, and then may suddenly stop and remain stable for life. In this type, whitening of the hairs (leukotrichia) is very common.

The most commonly affected areas in non-segmental vitiligo are:

Backs of the hands

Wrists

Elbows

Knees

Around the eyes

Around the mouth

Armpits

Genital area

Ankles

Which Methods Are Used to Make the Diagnosis and Is Testing Necessary?

Most of the time, an experienced eye and a detailed physical examination are sufficient to diagnose vitiligo. Its characteristic milk-white color and sharp borders reveal the disease to us. However, sometimes, especially in very fair-skinned people or in the very early stages of the disease, it may be difficult to distinguish the spots from normal skin. Or it may be confused with other conditions that cause white spots, such as fungal diseases.

In such cases, we benefit from a special light source called a Wood’s Lamp. When we shine this purple light on your skin in a dark room, if there is vitiligo, those areas take on a bright, glowing chalk-white color. This examination also enables us to see pigment losses that have begun in the lower layers of the skin, which we cannot yet notice with the naked eye.

After making the diagnosis, since we know that vitiligo is “not a disease that wanders alone,” we request some blood tests. Because vitiligo is an autoimmune disease, and if the immune system is attacking one place, it may also be attacking other organs. Therefore, it is necessary to draw the patient’s general health map.

The requested routine tests generally include the following:

Complete blood count

TSH (Thyroid-stimulating hormone)

Free T4

Anti-TPO antibody

Anti-TG antibody

Vitamin B12 level

Folic acid level

Fasting blood glucose

Ferritin level

Vitamin D level

Contact Us for Anything You Wonder About Skin Health!

 Contact via WhatsApp

How Is It Determined Whether the Disease Is Active or Not?

When starting treatment, the most critical question we ask ourselves is this: “Is the disease currently asleep (stable), or is the fire still going on (active)?” We shape our treatment plan entirely according to this. If the disease is active, that is, if the spread is continuing, our priority is to stop the spread. If it is stable, our priority is to restore the lost color.

The most important clue showing that the disease is active is the Koebner Phenomenon. This is the formation of a new vitiligo spot in an area of trauma on the patient’s skin (for example, a cut, burn, insect bite, or friction due to wearing a tight belt). If whitening occurs on your skin where you hit yourself, it means your disease is active and your immune system is on alert. In addition, the appearance of small confetti-like white dots on the edges of existing spots or blurring of the edges of the spots is also a sign of activity.

The signs of the active period of the disease are:

Koebner phenomenon

Confetti-like depigmentation

Growth of existing spots

Appearance of new spots

Redness at the edges of the spots

Itching in the spot areas

Three-color appearance (trichrome vitiligo)

In Which Situations Are Creams and Topical Treatments Used?

If the disease affects a small part of your body, that is, if there is vitiligo in a limited area, our first weapon is generally topical treatments, that is, medications applied to the skin. The aim here is both to stop the immune attack in that area of the skin and to encourage melanocytes to produce pigment again.

Cortisone Creams (Topical Steroids): They are especially very effective during the active periods when the disease has just started and there is mild redness or itching on the skin. Their responses on the face and neck area are quite good. However, these creams are like a double-edged sword. If they are used for a long time and uncontrollably, they may cause thinning of the skin, capillary cracking, and acne. Therefore, we generally apply “intermittent treatment”; for example, applying for two weeks and taking a break for one week.

Calcineurin Inhibitors: This group of medications (tacrolimus, pimecrolimus) does not contain cortisone. Therefore, they do not have side effects such as thinning of the skin. Thanks to this feature, they can be used safely and for a long time in areas where the skin is thin and sensitive, such as around the eyes, the face, the armpits, and the genital area. Their effectiveness increases much more especially when they are used together with light therapy.

The main agents used in topical treatments are:

Mometasone furoate

Methylprednisolone aceponate

Tacrolimus

Pimecrolimus

Clobetasol propionate

Calcipotriol

What Are Immune-Regulating Pills and Oral Mini-Pulse Treatment?

Sometimes the disease spreads so quickly that just applying cream is as insufficient as pouring a bucket of water on a forest fire. If the patient develops a new spot every day, a more radical and systemic intervention is required. At this point, we bring “Oral Mini-Pulse” (OMP) treatment into play.

In this treatment protocol, patients are given a systemic cortisone tablet on only two consecutive days of the week (for example, Saturday and Sunday mornings). The patient does not use medication on the remaining 5 days of the week. The purpose here is this: with the dose given on the weekend, the immune system is given a “stop” command and a shock effect is created, but since no medication is given during the week, the patient is protected from the long-term side effects of cortisone (such as weight gain, osteoporosis, blood pressure). This treatment usually lasts for 3 to 6 months, and its main goal is not so much to restore the color, but rather to stop the course. After the fire is extinguished, other treatments are started.

How Is Light Therapy (Phototherapy) Applied and Is It Harmful?

When vitiligo is widespread on the body, that is, in situations where applying cream one by one is not practical, the most effective and safest treatment method today is Narrow Band UVB (NB-UVB) phototherapy. First of all, let us make this clear; this procedure is not the same thing as cosmetic tanning beds. Tanning beds emit a broad wavelength and carry a risk of skin cancer. In medical phototherapy, only a very narrow part of the light with healing properties, 311 nanometers, is filtered and used.

The logic of the treatment is this: in our hair follicles, there are melanocyte stem cells waiting in reserve and sleeping. This special light both suppresses the immune system cells in the skin, stopping the war, and also tickles and awakens these sleeping cells in the hair follicles. The awakened cells multiply and migrate toward the white area, starting to color it again.

The patient enters a special cabin 2 or 3 times a week. The first sessions last seconds, then the dose is increased gradually. The formation of a slight pinkness (erythema) during the treatment shows that the dose is sufficient. It is necessary to be patient for at least 3 months to see the first results. While the face and trunk respond very well, the ends of the hands and feet may be a little more resistant.

For smaller and more stubborn areas, Excimer Laser or light systems are used. These devices direct high-energy light only onto the spot and do not touch the healthy skin. In this way, much higher doses can be given without burning the healthy skin, and healing accelerates.

The phototherapy methods are:

Whole-body narrow band UVB

Local narrow band UVB

Excimer laser

Excimer light

PUVA treatment

Contact Us for Anything You Wonder About Skin Health!

 Contact via WhatsApp

How Did the New Generation JAK Inhibitors Create a Revolution in Vitiligo Treatment?

The development that has created the greatest excitement in the medical world regarding vitiligo in recent years is JAK inhibitors. Scientists discovered which molecular signals (the JAK-STAT pathway) are used inside the cell on the path leading to the destruction of melanocytes in vitiligo. Drugs that block this signaling pathway were developed.

The cream forms of molecules such as Ruxolitinib have produced very successful results, especially in vitiligo on the face. These creams block the messenger carrying the immune system’s “attack” order. Thus, the pressure on melanocytes is lifted and pigment production begins again. In very widespread cases resistant to other treatments, oral forms of these drugs (tofacitinib, upadacitinib, etc.) can be used. However, these drugs are still very new, their costs are high, and blood values need to be monitored during use. Nevertheless, they are a turning point that has changed the concept of hopeless cases in vitiligo treatment.

Who Can Receive Surgical Treatment and Melanocyte Transplantation?

Can every vitiligo patient undergo surgery? No. The most important condition for surgical treatment is “stability.” In other words, the patient must not have developed any new spots for at least 1-2 years and the existing spots must not have enlarged. In addition, the Koebner phenomenon must be negative. If we perform surgery while the disease is active, both the transplanted cells will not take and new vitiligo will form in the healthy area from which we take the skin.

In suitable patients, especially in segmental vitiligo resistant to medication, surgery can create miracles. The most modern method is the Autologous Melanocyte Transplantation (Cellular Suspension) method. In this procedure, a thin piece of skin the size of a scale is taken from a non-visible area of the patient, usually such as the hip. In the laboratory, this piece of skin is separated into its cells through special procedures and turned into a liquid “cell cocktail.” Then the vitiligo area is lightly abraded with a laser, and this cocktail is sprayed there. The area is covered with special tapes.

The advantage of this method is that with the cells obtained from a piece of skin the size of a matchbox, we can cover an area the size of two hands. The color match is perfect because the pigment is the patient’s own pigment.

The applied surgical methods are:

Punch grafting

Suction blister epidermal grafting

Autologous melanocyte suspension

Hair follicle transplantation

Thin skin grafts

What Is Depigmentation Treatment and Who Is It Suitable For?

Sometimes vitiligo becomes so widespread that more than 80% of the body turns white. The remaining 10-20% of normal skin pieces, on the other hand, may stand out like dark spots on the face or hands and bother the patient cosmetically very much. In this case, instead of “trying to cover the white,” it may be a more logical option to “erase the remaining color.”

This procedure is called depigmentation. By using special mixtures containing monobenzone, the remaining healthy melanocytes are also destroyed, and the skin attains a completely uniform porcelain-white color. This procedure is irreversible; that is, there is no going back. Patients who make this decision need to protect themselves from the sun very strictly for life because they no longer have any natural protection left.

What Are the Nutrition and Lifestyle Recommendations for Vitiligo Patients?

The question our patients ask most frequently is “What should I eat, what should I not eat?” There is no miraculous food for vitiligo about which we can say “if you eat this, it will go away.” However, some nutritional principles are important to keep the immune system balanced and to fight oxidative stress. Eating a diet rich in antioxidants, colorful, and varied reduces cell damage.

Protection from the sun is not a preference but a necessity. Vitiligo areas are defenseless against sunburn. Sunburn not only causes pain, but may also trigger the spread of the disease (Koebner phenomenon). Therefore, sunscreen should be applied to exposed areas when going outside, regardless of summer or winter.

Psychological support is also an inseparable part of treatment. Stress triggers vitiligo, and vitiligo triggers stress. In order to break this vicious cycle, receiving professional support, practicing relaxation techniques, or taking up hobbies directly increases the success of the treatment.
